305.01 Declaration of Citizenship or Alien Status (AAM)

Each assistance group (AG) member must declare on the application for assistance the citizenship or alien status of each household member applying for assistance. Adults must sign for individuals under 18 years old.

Exceptions:

·         If there are no adult members in the AG, the child applying may sign for all members.

·         An authorized representative may sign for an adult applicant with a severe physical or mental disability who cannot provide a signature. If able to sign, however, the applicant's signature or mark must appear on the form. An "X" mark must be witnessed.
