Request assistance payments to be returned to the Division Family Assistance (DFA) when a financial assistance recipient dies and the payments:

• Are issued after the death of a recipient;

• Are the unspent portion of previously issued benefits remaining in the EBT cash account at the time of death;

• Were deposted to a deceased recipients personal account through EFT and which have not yet been spent to cover outstanding expenses of the deceased recipient (See Section 701.01 , Outstanding Expenses Covered by Last Payment);

• Were issued via a paper check and the check remains uncashed at the time of the individuals death; or

• Were issued via a paper check and the check cannot be delivered.

Make the request for return of assistance payments to:

• the administrator of the deceased recipients estate,

• the joint owner of the EFT account,

• any person with access to the deceased recipients EFT account, or

• any person in possession of, or with access to, the deceased recipients EBT cash account, such as an Authorized Representative.

For any financial assistance check uncashed at the time of the recipient's death, complete Form 86, Receipt Description.
