121.01 Interview Sites SR 09-07, 04/09 (AAM-A)

Conduct a personal interview in the office. The place where the interview is conducted must preserve the privacy and confidentiality of the interview.

Applicants who cannot return to the District Office for the scheduled interview due to transportation barriers or an ill family member may be interviewed by phone instead, at the scheduled time of the interview. A DFA supervisor must authorize the use of a phone interview over a personal interview, but all other interview requirements remain unchanged.

Interview an authorized representative if the individual cannot participate in the interview because of age, incapacity, illness, transportation difficulties, employment, language barriers, or similar reasons.

A home visit may be conducted instead of a personal interview in the office, if any of the following exist:

• the individual is unable to come to the District Office due to illness, incapacity, or lack of transportation;

• questionable information regarding eligibility criteria requires a more extensive investigation of circumstances; or

• the DFA Supervisor determines a home visit to be the most effective means available to conduct the interview.
