155.01 Timely Notice SR 02-24, 10/02 (AAM-A)

Inform each individual of the regularly scheduled redetermination on Form 807, Interview Appointment Notice. Form 807 advises the individual of their scheduled eligibility interview. New HEIGHTS automatically sends Form 807 to the individual in the month prior to the month the redetermination is due.

Personal Interview Required

If a personal interview is required (see Part 121, Interviews), a personal interview is scheduled by New HEIGHTS.

Personal Interview Not Required

If a personal interview is not required (see Part 121, Interviews), State Office sends the individual one of the following:

• Form 771A, Nursing Home Redetermination Form, if the individual resides in a nursing facility; or

• Form 800R Review for Continued Eligibility for Financial, Medical, and Food Stamp Benefits.

Form 800R is mailed to:

• AP/MA, and MA-only cases, without food stamps;

• In and Out cases with or without food stamps; and

• AP/MA and MA-only cases with related food stamp cases having no earned income and with all AP/MA and FS case sizes equal.
