During the application and redetermination process, individuals have primary responsibility for providing verifications needed to verify eligibility or compliance with an eligibility requirement. However, if an individual is reporting a change prior to their regularly scheduled redetermination, verification requirements are different.

 

For discovered or reported changes prior to the regularly scheduled redetermination, see PART 145 CHANGES.

 

At applications and redeterminations:

       Individuals are informed of what verifications are needed to determine eligibility;

       Individuals are allowed up to 10 calendar days to comply or to advise DHHS that the requested information cannot be obtained;

       Individuals can ask for assistance in identifying alternate methods of verification such as third party collateral contacts, when the information originally requested cannot be obtained timely or sufficiently and the individual has made reasonable efforts to obtain the missing proof; and

       All information provided to DHHS is evaluated to ensure the accuracy of the documentation.

 

Requested verifications can be submitted to DHHS in person, through the mail, or online via NH EASY.
