111.03 AR Signature Required (FAM)

The authorized representatives (AR) signature is required on BFA Form 778, Authorized Representative Declaration.

The AR must also acknowledge they have assisted the individual, by signing all forms they have assisted in completing.

If able, the individual must also sign or mark each form.
