When information or verification is needed to complete the eligibility determination, the individual is allowed 10 calendar days to provide it.

 

AE0026, Verification Checklist, explains what information or verification is needed and the date the information or verification is due back to the District Office.  The checklist is automatically generated anytime something is entered into New HEIGHTS and marked "Not Yet Verified," and eligibility is run.

       If New HEIGHTS is unavailable, the paper backup Form 808, Proof Needed to Determine Your Assistance, is used.

       If the individual does not provide the information or verification by the due date, the application is denied by the 30th day of application.

Exceptions:

       When you have split assistance groups (AG) (ex: mom and daughter applying for NHEP and a nephew applying for FAP) specific eligibility verification(s) that are not received, will not affect the eligibility of the entire case but only for the AG who failed to provide

•       When the specific eligibility factor in question does not affect the eligibility of the entire AG, disqualify the ineligible member(s) and open the remaining members. See FAM 211, COMPOSITION OF ASSISTANCE GROUP.

       If the AG is being denied due to lack of verification and the AG provides all the required verification within 30 days of the date of the denial notice:

-    The individual will fill out BFA Form 800, Application for Assistance, with their name, address, program of assistance, and signature;

-    The case is reviewed to ensure that no other changes have occurred that would affect eligibility and that no new programs have been requested on BFA Form 800 other than those which were originally denied; and

-  If the individual continues to meet all program eligibility requirements, financial assistance is generated in accordance with PART 123, BENEFIT DELIVERY TIME FRAMES.
