309.07 Verification (FAM)

A determination of eligibility may be based on the individual’s SSN card, SSA Form 5028, Receipt of Application for SSN, or Form SSA 2853-OP3, Message from Social Security.

All SSNs furnished by applicants must be verified through an electronic Social Security crossmatch. If the crossmatch does not yield verification, notify the applicant that they must provide the information required to verify the SSN or must apply for a replacement SSN card through the nearest SSA office.

Not meeting SSN requirements results in denial of benefits to the entire financial assistance group*. See Section 611.05 , Income of Disqualified Individuals.
