Request assistance payments to be returned to DFA when they:
• Are issued after the death of the recipient;
• Are the unspent portion of previously issued benefits remaining in the EBT cash account at the time of death;
• Were deposited to a deceased recipient’s personal account through EFT and which have not yet been spent to cover outstanding expenses of the deceased recipient (see FAM 705.01 , Outstanding Expenses Covered by Last Payment);
• Were issued via a paper check and the check cannot be delivered; or
• Were issued via a paper check and the check remains uncashed at the time of the individual’s death.
Make the request for return of assistance payments to:
• The administrator of the deceased recipient’s estate;
• The joint owner of the EFT account;
• Any person with access to the deceased recipient’s EFT account; or
• Any person in possession of, or with access to, the deceased recipient’s EBT cash account, such as an Authorized Representative.
For any assistance check uncashed at the time of the individual’s death, complete Form 86, Receipt Description.
