127.01 Interview Sites SR 94-04, 04/94 (FAM-A)

Conduct a face-to-face interview in the office. The place where the interview is conducted must preserve the privacy and confidentiality of the interview.

Exception: Interviews for Medical Assistance programs conducted by contract agency clinic staff, will be held in locations deemed appropriate by the agency.

Interview an authorized representative if the individual cannot participate in the interview because of age, incapacity, illness, transportation difficulties, employment, language barriers, or similar reasons.

Conduct a scheduled home visit if questionable information regarding eligibility criteria requires a more extensive investigation of circumstances, or if the DFA supervisor determines a home visit to be the most effective way to conduct the interview.

Waiver of the face-to-face interview does not exempt the individual from any program or verification requirements. Document in the case record why the office interview was waived.
