Examples of Acceptable Proofs of Age:
• copy of birth or baptismal certificate;
• school records with date of birth;
• hospital birth records;
• adoption papers;
• naturalization papers;
• third-party statement (e.g., statement from physician, school, etc.);
• immunization records; or
• an entry in the family Bible.
