Verify identity using any document that reasonably establishes the individual’s identity. If no document is available, use a collateral contact.
Exceptions:
• If an individual is unable to provide verification of identity, grant financial and/or medical assistance until the next regularly scheduled redetermination.
• For financial assistance for newborns, collateral contacts are acceptable instead of a birth, baptismal, or other formal record only for 1 year. If no birth record is available after 1 year, terminate financial assistance for the newborn.
Examples of Acceptable Proofs for Identity:
• library card;
• third-party statement (e.g., physician, school, etc.);
• documentation submitted as proof of age or citizenship;
• correspondence or utility bills in the individual’s name; or
• hospital record of birth.
