305.03 Declaration of Citizenship SR 94-04, 04/94 (FAM-A)

Form 752, Affidavit of Citizenship/Alien Status, is required of each household applying for assistance. The purpose of the form is to declare that each household member is a citizen or lawfully admitted alien. A new member must also complete Form 752 prior to being included in the household.

The form must be filled out by an adult member of the assistance group.

Exception(s):

• If there is no adult member in the assistance group, the child applying may sign for non-adults.

• An authorized representative may sign for an individual with a severe physical or mental disability who cannot provide a signature. If the individual can sign an "x," the mark must be witnessed.
