808.21 Employability Plan SR 04-26, 10/04 (FAM-A)

The Employment Plan (EP) is an individual plan designed to specify the steps the participant will take toward long-term economic independence and self-sufficiency. The NHEP team member develops the EP with the individual to identify:

• employment goals and appropriate NHEP activities for achieving the goals;

• problems/barriers to employment and action steps for problem-resolution; and

• assistance by NHEP, including needed support services to be authorized by NHEP.

The NHEP team member completes an EP for each NHEP participant. Form NHEP228, Employment and Family Self-Sufficiency Plan, *is the form upon which the EP is recorded.

Form NHEP228 may be used at any time.

EP Requirements

An EP must be completed for each NHEP participant. The EP is developed from information collected during the * assessment interview. The EP must contain the following:

• a description of employment goals for achieving long-term economic independence and self-sufficiency, including short-term goals for immediate employment;

• a description of the NHEP activities to be completed;

• the action steps for the participant and the NHEP team member;

• related time frames necessary for the participant to achieve the employment goals, prepare to obtain employment and resolve any serious and substantial personal barriers to employment; including time frames for NHEP assistance;

• the number of hours participating in the NHEP activity;

• the type and frequency of the contact between the NHEP individual and team member; and

• the support service needs of the individual and how those needs are being addressed.

When developing the EP, the team must take into consideration the following:

• available program activities and support services as funding and resources permit;

• local employment opportunities;

• the individuals skill level and aptitudes;

• to the extent possible, the employment preference of the individual; and

• any community activities or activities mandated by another agency in which the individual is currently involved.

The EP must: be reviewed and updated, if needed:

• at each individual contact;

• whenever changes in activities or services occur;

• whenever the individual is failing to make satisfactory progress in an activity; and

• whenever the individual requests a review.

The NHEP team member has final approval authority of the EP.
