901.01 Filing an Application SR 01-02, 05/01 (FAM-A)

To file an application for child care services, the individual must comply with all of the following:

• file an official application for assistance at a district office, NHEP team, or contract agency;

• provide required verification of eligibility criteria; and

• participate in a face-to-face eligibility interview.

 

Exception: *12 month EMA recipients whose eligibility for EMA is based on loss of TANF financial assistance due to new or increased earnings or hours of employment of the principal wage earner and who were receiving child care assistance at the time the TANF cash case closed are not required to file a new application*.

Staff must provide Form 249, Helpful Hints for Child Care Assistance, to each child care applicant.
