903.07 Required Verification to Grant a Good Cause Extension SR 17-12 Dated 07/17 (FAM-A)

If the family meets the good cause criteria for failing to be linked to a provider within the required 30 calendar days from the date of application or 30 calendar days from when the child was released from the waitlist, verify by the following means:

If the family is unable to find child care with the assistance of the Child Care Resource & Referral Agency during the 30 day period, require verification from the Child Care Resource & Referral Agency. The verification must state that the parent has been searching for child care with the assistance of Child Care Resource & Referral and child care for the specific child has not been identified. There will be no extensions beyond the additional 30 calendar days.

If the parent or child has had an in-patient hospital stay during the 30 day period, require the hospital record, the discharge statement or a statement from the attending physician.
