903.07 REQUIRED VERIFICATION TO GRANT A GOOD CAUSE EXTENSION SR 14-34, 11/14 (FAM-A)

If the family meets the good cause criteria for failing to be linked to a provider within therequired 30 calendar days from the date of application or 30 calendar days from when the child was released from the waitlist, verify by the following means:

• If the family is unable to find child care with the assistance of the Child Care Resource & Referral Agency during the 30-day period, require verification from the Child Care Resource & Referral Agency. The verification must state that the parent has been searching for child care with the assistance of Child Care Resource & Referral and child care for the specific child has not been identified. There will be no extensions beyond the additional 30 calendar days.

• If the parent or child has had an in-patient hospital stay during the 30-day period, require the hospital record, the discharge statement or a statement from the attending physician.
