111.03 AR Signature Required (FSM)

The Authorized Representative (AR) assisting the individual must sign the Form 800HR, New HEIGHTS-generated Application for Assistance, or DFA Form 800, Application for Assistance, and any other forms the AR assisted in completing, including the New HEIGHTS-generated Form 811H, Eligibility Interview Summary (EIS). If able, the individual must also sign or mark each form.

The AR assisting the individual must sign DFA Form 778, Authorized Representative Declaration.
