145.09 Notifying Individuals of Reporting Requirements (FSM)

At each eligibility interview, notify the individual of the requirement to report changes, what changes must be reported, how to report changes, and when to report changes and provide the individual with BFA Form 215, Reporting Requirements Handout.

If the household is eligible for a 4, 5, or 6-month certification period, explain the special reporting requirements and highlight on BFA Form 215 the gross monthly income amount for the household size. See Section 145.04, Special Reporting Requirements.

Explain BFA Form 470, SNAP Change Report Form, to the individual at each eligibility interview.

Give the individual a new BFA Form 470 and a self-addressed stamped envelope when any of the following situations occur:

• the individual is certified or recertified for eligibility; or

• a change is reported:

- via BFA Form 470 or other mail;

- in person; or

- via telephone.

BFA Form 470 does not need to be mailed to the individual if a change is reported online via NH EASY.
