161.03 Required Verification for Recertification (FSM) |

Notify the individual in writing of any required information or verification needed to complete the recertification, and the due date for returning the information or verification.
New HEIGHTS will generate a verification checklist that includes the information/verification that is needed and the due date by which it must be provided.
If New HEIGHTS is unavailable, use the paper backup BFA Form 808, Proof Needed to Determine Your Assistance.
The individual must provide verification only of information that is incomplete, inaccurate, inconsistent, outdated, changed since the last eligibility determination, or otherwise questionable.
DO NOT re-verify deductions incurred by the recipient when:
• there have been no changes in the deductions; and
• the deductions have been previously verified by a third party.
Assist individuals in obtaining missing verification when:
• they are unable to provide missing verification due to circumstances beyond their control; or
• a request for information was sent or given to a third party and that party fails or refuses to cooperate.
If unsuccessful:
• document the case record indicating the household’s inability to obtain verification; and
• allow the individual to submit a written self-declaration statement to verify the missing information.
The individual must provide verification of all of the following:
• changes in income, including termination of employment, if the source changed, or if the amount changed by more than $50 per month;
• new medical expenses, or changes in total recurring medical expenses of more than $25 per month;
• new social security numbers;
• work hours, if ABAWD and not exempt from work requirements; and
• other information which has changed, or unchanged information if the information is incomplete, inaccurate, inconsistent or outdated.
