145.05 How to Report SR 12-22 Dated 04/12 (FSM-A)

Individuals may report changes in any of the following ways:

• online through the individual’s NH EASY account;

• by mail;

• by telephone;

• in person;

• through someone acting on the individual’s behalf; or

• on DFA Form 470, Food Stamp Program Change Report Form.
