Inform each individual of the need for a redetermination in order for benefits to be continued.
Notify individuals that they are required to participate in a face-to-face interview redetermination/recertification or mail redetermination based on their household's circumstances.
Face-to-Face Interview Redetermination/Recertification
Use Form 807, Interview Appointment Notice, to notify individuals required to participate in a face-to-face interview redetermination/recertification. Form 807 advises the individual:
. that an eligibility interview has been scheduled for them;
. that they must call to reschedule if they are unable to attend the appointment and wish to continue receiving benefits; and
. the effect their failure to attend may have on their benefits.
Form 807 is sent to the individual in the month prior to the month the redetermination is scheduled, along with:
. Form 775, Rental Verification Request; and
. Form 756, Employment Verification Request.
Exception: In order to provide a timely recertification of benefits for households certified for 1 or 2 months, complete paper Form 807 and give or send to the applicant at the time of certification.
Mail Recertification
Adult financial/medical cases with related food stamps, that meet criteria in AAM 121, may participate in the mail recertification process. Those assistance groups participating in a mail recertification must be granted a face-to-face interview if one is requested by the household or by the authorized representative.
The mail recertification package is sent to eligible cases by the 15th of the month prior to the month the redetermination/recertification is due and includes:
. Form 800R, Review for Continued Eligibility for Financial, Medical, and Food Stamp Benefits; and
. a stamped return envelope.
Form 800R:
. advises the household to return the application form and verifications by the 15th of the following month;
. informs the household that application forms filed after the 15th of the following month may result in a delay of benefits; and
. advises the household to contact the District Office if they have any questions.
