Inform each individual of the need for a redetermination in order for benefits to be continued.
Notify individuals that they are required to participate in a face-to-face interview redetermination/recertification or mail redetermination based on their household’s circumstances.
Face-to-face Interview Redetermination/Recertification
Use Form 807, Interview Appointment Notice, to notify individuals required to participate in a face-to-face interview redetermination/recertification. Form 807 advises the individual:
• That an eligibility interview that has been scheduled for them;
• That they must call if they are unable to attend and wish to continue receiving benefits; and
• The effect their failure to attend may have on their benefits.
Send Form 807 to the individual in the month prior to the month the redetermination is scheduled.
Mail Recertification
Individuals participating in a mail recertification must be granted a face-to-face interview if one is requested by the household or by the authorized representative.
The appropriate mail recertification package will be sent to eligible cases by the 15th of the month prior to the month the redetermination/recertification is due.
KidsCare PLUS medical coverage recipients with food stamps, and adult financial/medical cases with related food stamps that meet criteria in AAM 121, may participate in the mail recertification process.
Exception: Children with Severe Disabilities - Home Care (CSD-HC) recipients who also receive food stamps are not eligible to participate in the mail recertification process.
Notification to individuals who are going to be participating in the mail recertification process is accomplished when the following material is sent:
• Form 800R(Adult), Application for Redetermining Eligibility for Assistance,
• Form 800C(Adult), Cover Letter for Form 800R(Adult),
• Form 775, Rental Verification Request,
• Form 756, Employment Verification Request, and
• a stamped return envelope.
Form 800C(Adult) advises the household to return the application form and verifications within 10 days, and informs the household that application forms filed after the 15th of the month may result in a delay of benefits.
