The individual must complete and return by the 15th day of the last month of the certification period for each redetermination/recertification:
• Form 800H, HEIGHTS Application Form or paper Form 800, Application, and participate in a face-to-face interview or
• Form 800R(Adult), Application for Redetermining Eligibility for Assistance, for mail redeterminations to receive uninterrupted benefits and prevent delays in the recertification process.
• Send Form 808, Proof Needed to Determine Your Assistance, when verification is not sufficient or not provided in the Mail recertification process.
• Allow individuals 10 additional days to provide the needed verification.
Require face-to-face interviews with individuals who do not fully complete either Form 800H, paper Form 800, or Form 800R(Adult), Application For Redetermining Eligibility For Assistance, but at a minimum provide their name, address and sign the form.
Contact the individual to ensure information has not changed since the recertification appointment if the case is not being processed within one month of the appointment date.
Process recertifications to ensure uninterrupted benefits if the individual has participated in an interview and provided required verification in time to be included in the normal issuance cycle.
Issue benefits to recertified individuals according to the following time frames:
• By the 1st day of the following month when a household provides verification by the end of the month.
• No later than 5 days after receipt of verification, when the individual has participated in an interview and provided required verification within 30 days after the application date, but too late to be included in the normal issuance cycle.
• Immediately, when the individual has completed the recertification process, but the Division of Family Assistance has not determined eligibility in time to provide benefits in the individual’s normal issuance cycle. Restore any benefits lost due to the Division’s error.
Process recertifications/redeterminations within 30 days of the application date if the individual has not participated in an interview or provided required verification by the end of the certification period. See Section 161.07, Termination at Redetermination/Recertification.
Exception: Complete Form 808, Proof Needed to Determine Your Assistance, if the individual fails to provide adequate proof or there is missing verification. Allow the individual 10 additional days to provide any missing verification.
Do not close or deny Food Stamp households when they are unable to obtain proof and contact the District Office on or before the last day of the month of the current certification period, or within the additional 10 day time frame provided for by Form 808.
