109.07 Reuse of Forms 800, 800HR, and 800MA After Denial or Closing (MAM)

Reuse DFA Forms 800, New HEIGHTS-generated 800HR, or DFA Form 800MA to open a denied application or to reopen a closed application if the denial or closing was:

• an error; or

• due to failure to provide verification, and the following applies:

- the applicant provides required verification within 30 days of the denial notice; or

- a former recipient provides required verification within 10 days of the last day of the advance notice period.

If the information on the DFA Form 800, New HEIGHTS-generated 800HR, or DFA 800MA has changed, the individual must provide verification if required, and the worker must document the changes. Conduct an interview if necessary.
