301.13 Verification of Residency (MAM)

Self-attestation of residency is accepted for medical assistance with no further proof required, unless conflicting evidence is present.

If conflicting evidence is present, request evidence of residence, which may include (but not be limited to) a rental agreement or rent receipt; utility bill; library card; vehicle registration; or a third-party statement, such as from a school or a healthcare provider.
