303.01 Declaration of Citizenship or Non-Citizen Status (MAM)

One adult representative of a household applying for assistance must declare on the application the citizenship/non-citizen status of each household member applying for assistance. Adults must sign for individuals under 18 years old.

Exceptions:

. If there are no adult assistance group members, the applicant may sign for all members.

. An authorized representative may sign for an applicant with a severe physical or mental disability who cannot provide a signature. If able to sign, however, the applicant's signature or mark must appear on the form. An "X" mark must be witnessed.
