• Accurate Information — Individuals are responsible for supplying complete and truthful information to meet eligibility requirements.

• Changes — Individuals are responsible for reporting all changes in circumstances which affect their eligibility.

• Compliance — Individuals are responsible for complying with the eligibility requirements of the assistance program(s) they apply for.

• Verification — Individuals are responsible for providing required verifications.

• Communication with DHHS — Individuals are responsible for reading and responding to all notifications received from DHHS.
