211.01 MEAD Medical Criteria SR 13-35 Dated 10/13

Medical eligibility for MEAD is determined by the Disability Determination Unit (DDU). See Department Administrative Rules at He-W 504 for further information.

To meet MEAD medical criteria, the individual must have a documented medical impairment that:

• is included in the Social Security Administration’s Listing of Impairments; and

• has persisted or is expected to persist for a minimum of 48 consecutive months.

MEAD medical eligibility determinations are not required for:

• blind individuals;

• individuals determined eligible for APTD or ANB within the 12 months prior to application for MEAD; and

• individuals who have not closed for APTD or ANB due to medical improvement during the previous 12 months.

For MEAD medical determinations, send the following forms when forwarding the request to the DDU:

• DDU Form 177, Non-Medical Evaluation of Disability; and

• DDU Form 900, Authorization to Release Information

If the individual does not meet MEAD medical eligibility criteria and wants to apply for ANB or APTD, a separate ANB or APTD medical eligibility determination is required.
