225.03 Required Verification SR 13-35, 10/13 (MAM-A)

The individual must provide verification of all FANF medical assistance eligibility criteria except the deprivation and presence of a dependent child. Additional required verification includes written verification of pregnancy and, when established, the number of fetuses.

• Require verification of pregnancy at initial application, each redetermination, and anytime the condition of pregnancy changes or becomes questionable.

• Accept a signed statement from a licensed physician, advanced registered nurse practitioner, or other licensed medical practitioner.

• Assume the presence of one fetus unless verification is provided to indicate more than one.
