229 CHILDREN WITH SEVERE DISABILITIES (CSD) SR 14-04, 01/14 (MAM-A)

*Children with Severe Disabilities (CSD) is a Medicaid eligibility category under the Children’s Medicaid category that uses Modified Adjusted Gross Income (MAGI) in the eligibility determination process. Applicants and recipients of CSD do not have to meet the specific FANF requirements of deprivation of support or care of a parent or living with a specified relative, to be eligible for CSD assistance. Application for CSD medical assistance is made online, on paper DFA Form 800, Application for Assistance, DFA Form 800MA, Application for Health Coverage & Help Paying Costs, or Form 800HR, HEIGHTS-generated Application for Assistance, if application for the program is being made by a recipient of another program during the individuals redetermination/recertification.

To be eligible, the child must:

• be under *20 years of age.

• have countable net income *available *to the child that does not exceed *196% of the Federal Poverty Levels (FPL).

• meet the Disability Determination Unit (DDU) disability criteria.
