301.13 Verification of Residency  (MAM-A)

SR 14-04 Dated 01/14

Previous Policy

____________________________________________________________

How residency is verified depends upon the whether the individual is applying for or receiving non-modified adjusted gross income (MAGI)-related medical assistance or MAGI-related medical assistance.

Non-MAGI-Related Medical Assistance

Verify New Hampshire residency before initial determination of eligibility, at redeterminations, upon learning the individual has moved, and if residency is questionable.

Use any documentation that reasonably establishes a New Hampshire address. The individual need not have a fixed living address as long as their New Hampshire residency can be verified.

Examples of Acceptable Proofs of NH Residency:

. lease, rental agreement, or rent receipt;

. electric, cable, heating fuel, or telephone bill;

. property tax bill;

. motor vehicle registration;

. library card or correspondence; or

. third-party statement (e.g., physician, school, etc).

Addresses such as post office boxes, rural route addresses, or general delivery addresses cannot establish New Hampshire residency.

Documenting the Address of Homeless Households in New HEIGHTS

For individuals or households that are considered homeless, do the following when entering the residential address into New HEIGHTS:

. Enter "homeless" as the residential address;

. Enter the DO's mailing address for the individual's or household's mailing address; and

. Enter the DO's name into the "Care Of" field.

The individual or household is responsible for retrieving their mail at the DO until the individual our household provides another mailing address.

MAGI-Related Medical Assistance

Self-attestation of residency is accepted, with no further proof required, for MAGI-related medical assistance.
