307.03 Completing an Application for an SSN SR 13-36, 11/13 (MAM-A)

 

All applicants who cannot furnish an SSN must apply in person for an original SSN or a replacement SSN card at the U.S. Social Security Administration (SSA).

Exception: Newborns may be enumerated at birth. Require SSA Form 2853-OP3, Message from Social Security, (used to confirm enumeration at birth at the hospital) to verify enumeration at birth.

Provide the applicant (unless the applicant is a newborn) with SSA Form SS-5, Application for Social Security Card, and advise the individual to submit SSA Form SS-5 to the nearest SSA office and request SSA Form 5028, Receipt of Application for SSN. Advise the applicant that benefits cannot be issued until verification of enumeration has been submitted or good cause for not enumerating has been determined to exist.

Application for an SSN will generate an original social security card. When an original social security card has been lost, application for an SSN will generate a duplicate of the original.

If more than one SSN appears for the same individual, report the duplication to SSA.

If providing the individual with SSA Form SS-5, Application for Social Security Card, complete the NPN block so that SSA can verify electronically the issuance of an SSN.
