RFA-2024-DPHS-05-CASEM: Case Management and Support Services for Individuals Living with Human Immunodeficiency Virus (HIV)

The New Hampshire Department of Health and Human Services, Division of Public Health Services, Bureau of Infectious Disease Control is seeking responses to this Request for Applications from qualified Vendors to provide Medical Case Management (MCM), Non-Medical Case Management (NMCM) and Human Immunodeficiency Virus (HIV) support services to individuals living with HIV and receiving services through the New Hampshire Ryan White Comprehensive AIDS Resource Emergency (NH Care) Program.
