NHCarePath Partner Resources - Consumer Booklets

The NH Balancing Incentive Program team is honored to have you as a valued partner of NHCarePath, and would like to provide you with resources to help you better succeed in this role.

The Consumer Booklets are designed to provide individuals with information on Medicaid waivers and home and community-based services. There are six booklets:
