NH Easy 777 Provider Enrollment Form
BDS is excited to offer the provider access enhancement to NH Easy. In order to access the NH Easy BDS Service Provider Management platform, all provider agencies must complete the NH Easy Provider enrollment form (777). The NH Easy Provider enrollment form (777), must be completed by the assigned NH EASY Administrator (one administrator per organization) by June 15th, 2024. Please inquire internally at your provider agency to identify who the NH Easy Administrator is. Detailed instructions on how to complete the form may be found within the NH Easy Provider Enrollment Memo.
Please submit this form to BDS@dhhs.nh.gov, with the subject line “Provider Access NH Easy 777 Form”.
The form indicates to submit it to DHHS.NHEASYHelpDesk@dhhs.nh.gov, however during this initial enrollment process please do not submit to that email address. Through June 15th, please submit to the BDS email as indicated above.

At least 5 business days advance notice is requested in order to assure availability; requests made fewer than 5 days prior to the event will attempt to be accommodated but cannot be guaranteed.

Becoming a Medicaid-Enrolled Provider in New Hampshire
Under federal regulations from the Centers for Medicare and Medicaid Services (CMS), providers must have the option to bill for services directly through the State’s Medicaid Management Information System (MMIS) or use a third-party biller.
All providers providing services, including all Area Agencies, for individuals under the Developmental Disabilities (DD) Waiver, In-Home Supports (IHS) Waiver, and/or Acquired Brain Disorder (ABD) Waiver must be a Medicaid enrolled provider, whether they intend to bill directly or not.
- Visit NH’s MMIS Health Enterprise Portal to submit an application
- Entities interested in being a trading partner will also need to go through the enrollment process
