SUMMARY

This Supervisory Release (SR) releases the obsoletion of BFA Form 800R[EFS], Application for Continued Eligibility For SNAP Benefits ONLY.

NEW HEIGHTS & NH EASY SYSTEMS PROCEDURES

New HEIGHTS and NH EASY will remove BFA Form 800R[EFS], Application for Continued Eligibility For SNAP Benefits ONLY and its Spanish translated version from the forms repository.

An HCR for New HEIGHTS generated letter CM0023, EFS Ending Letter, has also been edited and submitted to New HEIGHTS. Once this update is completed a New HEIGHTS System alert will be issued.

IMPLEMENTATION

This policy is effective upon release of this SR. The paper version of BFA Form 800R[EFS], Application for Continued Eligibility For SNAP Benefits ONLY, its Spanish translated version and its instruction sheet BFA Form 800R[EFS](i), shall be destroyed upon the release of this SR. A Certificate of Destruction has been included with this SR. Any 800R[EFS] received now or in the future should still be treated as they had been in the past until circulation of the form is completed.

CLIENT NOTIFICATION

No special client notification is planned or needed.

TRAINING

No special training is planned or needed, however training material will require updates.
